Assessments/interRAI

Carry out the interRAI Community Health Assessment (CHA) and other assessments with GoldCare's Assessment Information Manager (AIM). In addition to providing standard assessment templates, AIM allows you to create your own. All assessments can be accessed from AIM's integrated web interface, which provides a streamlined and easy-to-use format to electronically complete, update and track assessments.

• Data Collection: Assessment information is accessible from a single window, and sections can be completed in any order. You can also concurrently track the progress of multiple assessments.

• Outcome Scores: AIM can be configured to calculate outcome scores once an assessment has been completed. Outcome scores can be used collectively to track care trends and identify shared traits or significant disparities among your client and resident population.

• Assessment Protocols: AIM can be configured to trigger recommendations for care follow up based on assessment details. A list of factors to consider and an AP report can also be generated, which gives you a problem-oriented framework for care planning or further assessment.

• Care Plan Builder: A care plan is automatically suggested for the client based on data from the completed assessment. Simply review the suggested care plan and individualize it for the client as required.

• Personal Health Profile: AIM consolidates the client’s assessments, assessment protocols and outcomes scores into a single data sheet that also serves as the client’s assessment history.

Assessments can be conducted at the point-of-care using GoldCare’s Care Anywhere solution, which enables you to use your laptop to collect client information. This information is synchronized with the GoldCare server when you return to the office. Assessment data can then be consolidated for analysis, measurement and benchmarking using GoldCare’s Insight application, which provides aggregate reporting capabilities.
